well for starters you should take the dog out AFTER each meal, not before he eats and by putting him in the create right after he eats and drinks you are making him hold it in or have an accident

You seem to be doing a good a job but try to give lots of treats and praise when he goes outside. Never spank or scold him for going inside they don’t understand this. Also, not to be harsh but the dog can not live his entire life in a crate. So you will have to try to keep him out some too and give him attention other than a walk. Make sure your crate is the appropriate size as well. If it is too big then they will potty in it.

Why is he in the crate so often? If you are home, then you should leave him out of the crate and keep him close to you, a leash would work just fine

hate to tell you this but this is abuse. it is okay to crate train your pup, but too much is not healthy for them. It is in a puppy’s nature to explore and they need to be out and be able to socialize with you. It can cause joint and muscle problems in the little guy and in some severe cass the muscle damage is unrepairable. He definitely is going to whine. first he isn’t getting enough attention and second he has FEELINGS. Shoving him in a crate all day is just as bad as locking your toddler in their room all day. Crate time is needed but not over 12 hours a day. i read through your list and that poor puppy is in his crate over half of the day.
